**Pet Application: If you have pets, you will be required to complete pet registration before your rental application can be evaluated. All pets must be registered. First pet is $20 to register, additional pet $15. An Empire associate will contact you (via email) with more details once your application is being evaluated.

Last 2 addresses and Landlord Contact Information
Employer and Previous Employer: Name, Contact, Start Date, Salary
Emergency Contact Information
Have the following documents ready to UPLOAD when applying:
I.D. (drivers license, i.d. card, passport, etc.)
2 months of Paystubs or Income Verification
Copy of Renters Insurance
Pet Pictures (if applicable)
Current Vet Records (if applicable)
